Before completing the application form, please read the notes given below:

1. Applicants should read the enclosed Regulations and Conditions. Submission of an application will be taken to imply acceptance of these regulations.

2. Applicants should submit only complete forms and should arrange for these forms to be in a clear typescript.

3. All questions must be answered or marked " not applicable".

4. Applicants should return TEN COPIES of all parts of the application form by 5pm on the deadline to:

5. Application forms must conform to the presented layout and provide the necessary information with in the space(s) provided on the form; additional sheets (eg lists of publications) will not be included when the application is assessed.

6. It is the applicant's responsibility to ensure that all the relevant sections are completed before submitting the form.

Applicants are responsible for passing parts B, C and D of the application form to the appropriate authorities for completion and for ensuring TEN COPIES of each are returned.
